I have designed "The Maples MAP" or "My Action Plan" as a new way of defining the support needs of people with mental health problems or learning difficulties. It is easily accessible for  people of varying degrees of ability and designed to help put the emphasis on them taking charge of their own support needs. The aim is to enable support workers to initiate the sometimes difficult conversations about what the client needs or wants to achieve and how best they can get there with varying levels of help.
