i think satsuki and miles made the conscious decision to make sure that satoshi would be the only child in the royal!au

pregnancy, regardless of the verse, was fucking hell on satsuki. she didn’t enjoy it and i think in the royal!au? she almost died a few times (and thus almost lost satoshi). her labor was hell and it’s a miracle both she and satoshi made it out alive. 

so. satoshi, unfortunately, was to be the only one.
