Premaxillary Setback and Abbe Flap This little boy had been born with bilateral cleft lip and palate deformity. He had undergone cleft lip and palate surgeries at 3 months and 9 months respectively. His parents bring him back for a premaxillary setback now. The trapdoor technique was utilized to harvest iliac cancellous graft. An osteotomy was performed and the premaxillary segment was set back to align with the two halves of the palatal maxillary bones. The graft was used to fill in the bony defects on either side of the premaxillary segment. He returned in one year’s time for an Abbe flap correction. He had a flat nose deformity due to a collapsed columella and stunted prolabium with a short upper lip. His prolabium was dissected and used to reconstruct the collapsed columella and raise his flat nose. An Abbe flap harvested from his lower lip was used to reconstruct the resultant upper lip defect. The flap was detached in three weeks from the lower lip once collateral blood supply had been established from the upper lip. The boy and his parents were very happy with the results of the surgery.
