Ayano has known you ever since she started first grade. You being her only friend in school,and with time this friendship evolved.
Which made you and the dark haired girl grow closer.
Her behavior never made you uncomfortable around her, neither did her silence. Because in truth,you liked it better, the difference between you two matched perfectly.
Your out going and talkative personality tangling with Ayano's introverted and silent language.
Kids your age called her many names due to her lack of emotions,but every time you were there to send them on their way with a bruise or two, nobody talks to your best friend like that!
At first when she was little,the girl saw your behavior towards her unnecessary. Why were you trying to get close to her? She had nothing to offer back to you.
But as she grew, whenever you weren't around her there was this empty spot in her heart,and that dull feeling griping her soul.
Not long before starting highschool did she realize what this meant.
Her mother talked to Ayano about this,since she was a child.Of course you were her special person,the one who will fill her heart with joy and warmth.

It's confirmed that there have been some branches who took the husband's surname.
And!
it's been confirmed that most Aishi tend to lose contact with their families because the hereditary "Aishi condition" makes them apathetic to everything that's not their senpai, often isolating them from others. Once they "cure" themselves by finding their Senpai, they become distant viewing family as less important than their beloved
Both of these facts combined could mean that there are Aishi whose last names aren't Aishi and do not know they are related to those whose last names is Aishi

So my theory is he married an Aishi whose last names wasn't Aishi and she wasn't aware of her connection to the Aishi bloodline due to generations of losing contact

Well, I think she too is an Aishi, here's why:

This is solemnly my theory, but what about you guys?
A small fun fact about my yansim au, EVERY Aishi is not evil. They are born with no to very little emotions, it just so happens that the curse was taught to only harm people. A family tradition no one can fully break, no matter how hard they try. Ayano is the “second” start of %100 of the curse.
Also Kataba and Torahiko do love each other. They are not the most healthy (how can you be when your wife is a Yandere and you survived ww2?), but they do care for one another in a true way (another fun fact, Kataba may or may not have her actual “senpai”…).

As for the other Aishi members (that we know about at least. And Ayato does not exist.):
Ayano is obsessed with Taro. Taro thinks of Ayano as an acquaintance.
Yamara sees her “senpai” as living medicine. Her “senpai” doesn’t know.
Ryoba is obsessed with Jokichi. Jokichi loves his daughter but fears his wife.
Ryoba’s sister is obsessed with her “senpai”. Her senpai is dead. Ryoba's sister is missing.
Dozuki sees Sureibu as living medicine. Sureibu no longer can think for himself.
Kataba is obsessed and in love with Torahiko. Torahiko loves and fears his wife, and is feels regret for Dozuki.
Azebiki’s brother is a mystery.
Azebiki sees her “senpai” as living medicine but also human. Her senpai is tolerates her sickness. Azebiki despises her granddaughter and feels pity for Kataba.
Kugi is obsessed with her “senpai”. He fears his wife but loves both his children.
Hiki is dead. Yet Kugi always sees her in the corner of her eye.
The first Aishi is under the tree. Along with the Basu sisters mother.
