Headcanon: the doctor doesn't speak a lick of siracusan but uses context clues to overcome the language barrier most of the time.
So after the 'conversation' ends and Leontuzzo walks away, if someone asks what they were talking about the doctor just goes,
"uh, something about the siracusan mafia probably. Idk tbh"
And then when the time comes that they can't use context clues anymore they look Leontuzzo straight in the eye and go,
