All possible causes of death for Hashirama I considered, ranked
In the Naruto databook (2002) claims that "Hashirama lost his life when the village had just been founded," but this is outdated. It also doesn't say how he died, it only mentions that the world was engufled in war during that time, but it doesn't say he died in it. So I forgot it entirely. These are causes of death I considered for the last years.
1) Heroic sacrifice during a battle Likelihood: 10/10 Interesting: 3/10 This is the Occam's razor explanation, because Hashirama already has a pattern of throwing himself in front of danger. He tried to die for peace, he probably tried to die for anyone who looked remotely sad. It's boring because it's predictable, but predictable is often canon. Also this considering his missing armor plate.
2) Got drunk, went to pee, fell off a rock Likelihood: 4/10 Interesting: 5/10 Hashirama can be easily distracted, and he likes to have a cup of alcohol here and there. However, he also has regeneration, and reflexes. Unless he fell off a very big rock, into a river, or ravine, or both.
3) Suicide Likelihood: 5/10 Interesting: 6/10 He does have self-sacrificial impulses and he could have some escapist fantasies, however I don't believe he has a suicidal intent, he wants escape, not death. I don't believe he would die intentionally, but who knows.
4) He faked his death and became a tour guide in the old Senju Clan compound Likelihood: 0.5/10 Interesting: 8/10 Hilarious, but impossible, because his body was buried, his chakra signature would be known, Tobirama would notice instantly.
5) He turned into a forest scent Likelihood: 0.5/10 Interesting: 4/10 Same issue. Body present = no mystical disappearance, unless his body was a clone.
6) Illness from mokuton or regeneration Likelihood: 9/10 Interesting: 7/10 This is the second strongest theory by far. Rapid cell regeneration means a cancer risk. It's quiet, medical, tragic death that fits the canon.
7) Assassinated by another village/country (via violence) Likelihood: 4/10 Interesting: 6/10 Possible, but unlikely because Konoha would retaliate, and Hashirama's death would be considered a political event, and it would be talked about.
8) Assassinated by another village/country via poison Likelihood: 8/10 Interesting: 7/10 Poison that mimics illness could be hidden, covered up and chalked down to "natural causes." It's possible, his death is a mystery even for people in Konoha, that's why even readers don't know the cause.
9) Kyuubi took over Mito and killed him in his sleep Likelihood: 3/10 Interesting: 10/10 This is the darkes and most politically suppressed option. Since Kyuubi doesn't have the best relationship with Hashirama, a moment of weakness could be catastrophic. Tobirama would cover it up to protect Mito, and the village would rarely speak of it again. It also makes sense why Naruto became a pariah just because he had Kyuubi sealed inside of himself, because Kyuubi killed their beloved Hokage.
10) Killed by Madara or Kakuzu indirectly Likelihood: 7/10 Interesting: 5/10 It is possible that during one of his big combats he suffered injuries that weakened him, wrecked his health and caused long term damage. This is not that interesting to me, but it is certainly possible.
11) Environmental causes (a freak accident) Likelihood: 4/10 Interesting: 3/10 Environmental deaths are possible, because he often has to wander off or go to a mission. He could be crushed by a falling tree or he could be hit by a lightning strike. It is not dramatic, political, or mystical. It would just be a bad luck, it unfortunately happens.
12) Jutsu/chakra backlash Likelihood: 6/10 Interesting: 7/10 This is where Hashirama dies because his own technique fails. For example, he overuses hid regeneration, or chakra explodes in his face while he was trying to come up with a new technique.

Madara did in fact communicated to Hashirama what he was feeling, that he felt guilty for not being able to protect the ones he cared about (his siblings)
And Hashirama understood what Madara was going through, that he was ostracized by the village and his clan, that's why he wanted him to be hokage, to give him again the fulfillment he lost as someone who was a protector
And they drift apart bc Hashirama can't concile those things, he can't concile the village with Madara, which is not something impossible, he just let it be, he repressed it like a ticking bomb.
He also can't concile that Izuna's death is what allowed him to have his dream come true and have Madara back in his life. And Madara doesn't grow a resentment towards that, but he is disappointed that Hashirama can't keep his own word by trying to please everybody.
So when Hashirama points out Sasuke's resemblance to Izuna, it goes back to this moment where he knew Madara was a kind person, he knew that he shouldn't have been neglectful towards his feelings, but he still keeps banging his head against the wall not seeing the real problem, that is him not talking to Madara.
