sam's mind goes to 'family business; you stick with family, no matter what'. and perhaps that is part of it, because sticking with family is a big part of who dean is, and sam knows it. but I believe what dean meant here was broader, tying into the theme of knowledge coming with a duty to help. knowledge is power, power to help others when they are in need. if you know how to fight monsters threatening people's lives, dean thinks, then you should use that knowledge to do so. that's a responsibility to people, one that dean finds it hard to walk away from, and one that sam does walk away from multiple times
