So why does Ashley get called Lt. Commander when her and Shepard are technically the same rank? I dont hear anyone saying Lt. Commander Shepard.
Because Shepard is universally known as 'COMMANDER SHEPARD' both in-game and outside it for meta reasons. It has a ring to it, I guess. She's known as that. Plus Commander is short hand. Like people don't call Kaidan Staff Lieutenant in ME1 just Lieutenant even though that's not his full rank. It's just convenience and completely accepted. They probably call Ash that just to show her new rank off or something, or to differentiate her from Shepard's rank.

That's OK because she is the Commanding Officer of the Normandy (and has great authority in the war) and he still defers to her in those situations. Because *your* ship is a massive thing. You need to have permission to come onboard and everything. I think even higher ranked officers must ask.
There's a whole formality to it.The Normandy is Shepard's castle, her domain, no matter who's on it.
If they weren't on the Normandy and Shepard wasn't running her own ops, for example joined one of his, or was just as part of a crew? She would be the one who has to salute him and say
'No, sir.'
'Yes, sir.'
'Three bags full, sir.'
Just like she does Hackett or Anderson. I mean, it depends on the CO, or Officer, how much of a hardass they are. And she wouldn't do it when they're off-duty. But in front of other officers or enlisted? I betcha she would have to pull out the military etiquette and pretend she doesn't know what he looks like naked while she salutes him :P
