A 36-year-old female patient with blepharoptosis (droopy eyelid).
She also had upward slanting eyes.

I performed levator aponeurosis advancement (standard operation for blepharoptosis) 
I also performed lower eyelid descent procedure with lateral canthoplasty to make downward slanting eyes.

Many people including unskilled cosmetic surgeons think that lateral canthoplasty operation makes length of eye longer but it can not in fact.
I only perform lateral canthoplasty in order to strengthen lower eyelid descent procedure.

These photos were taken 5 days after the operations. The swelling both upper & lower eyelids seen in the photos taken just after the operations is getting better. There is conjunctival edema at the lateral canthuses.
Conjunctival edema is a frequent complication after lower eyelid and extraocular angle surgery, and it looks like jelly placed on the outer eye corner.
Conjunctival edema after the surgery usually disappears in 2 or 3 weeks even in the natural course. But in order to reduce the edema earlier, I frequently inject 5 or 10 times-diluted dexamethasone locally. I'll post photos taken 1 month aftert the operations tomorrow or sometime lator.

36歳女性の眼瞼下垂の患者さんです。
また、吊り目にも悩んでらっしゃいました。
挙筋腱膜前転法による眼瞼下垂の手術をおこないました。
また、吊り目をたれ目にする為に、下眼瞼下制と目尻切開も行いました。
多くの人（中には美容外科医ですら）目尻切開の手術で目の横方向の長さが長くなると考えていますが、実際にはそうはなりません。
私は下眼瞼下制の効果を高める目的のみで、目尻切開の手術を行っています。

この写真は手術5日後に撮影しました。上下のまぶたに見られた腫れは、手術直後の写真と比べ改善しています。

両目の目尻に、結膜浮腫（白目の腫れ。目尻側の白めにゼリーが付いているように見えます）が有ります。
結膜浮腫は下まぶたや目尻の手術後に頻繁に見られる合併症です。 自然経過でも2〜3週間で無くなりますが、より早く改善させる為に、5〜10倍希釈のデカドロンを局所注入する事が多いです。

後日、術後1ヶ月の写真もアップしますのでお楽しみに。

A 21-year-old female patient with blepharoptosis (droopy eyelid).

 She also had puffy eyelids. I performed levator aponeurosis advancement (standard operation for blepharoptosis) with ROOFectomy (resection of Retro-Orbicularis Oculus Fat) because her droopy eyelids were not caused only by the defect of the levator aponeurosis but also the thickness of the ROOF.
