It is the Snapdragon Tam by Ysolda Teague. I am in love with 95% of her designs, and this hat is no exception.

It was a one-off Halloween colorway from GnomeAcres. I can't find the label anywhere, so I can't really give you more than that. Sorry! I'll keep looking for the label, it might turn up.
