Someone actually wrote that..wow. That's..yeah. Any who, i am not sure if this will help, but this is why i assume the person meant. Mizuki is the doctor. While she is doing the surgery, she sees some sort of creature coming out of the patients brain

yeah i thought about it and im pretty sure she's the one doing the surgery but i think she also has some sort of medical condition that's being personified by the dragon sharp teeth thingy....?
