Mark passing the basketball off to Carter, calling it "your ball." The basketball HE has been throwing about since season 1; the basketball he's played with through good times and bad times, through friends and coworkers coming and going; the basketball he's taught Carter many life lessons through playing with.
He passes the ball off to the current chief resident of the ER, Carter, the position he was in when the show started and says "You set the tone, Carter" WHICH IS WHAT HE TOLD CARTER IN S1 AS A MED STUDENT WHEN HE WAS LOST AND WONDERING IF HE REALLY COULD BE AN ER DOC.
THATS HIS SON YOU'RE HONOUR.
The entire episode was filled with insanely good metaphors and imagery, but this final scene killed me. Quite literally passing the baton to Carter.
