You should make 2 medic ocs who are enemies but in the "i dig 2 graves before i go and kill that guy" way. In the way that one of them did something unforgivable to the other, and after the guy who was previously a victim gets revenge, they're now essentially guilty of the same thing. An eye for an eye leaves the whole world blind medics. Slightly homoerotic in a way that nothing could ever come of it, but the tension is still there.
They always hated each other. To the point, they would rip each other apart and turn into creatures. But... Years after years of fighting each other. Dr. Claw would develop unrequited love for Dr. Ripper.
