Patty Winsa writes: "Now, hospitals like Sunnybrook, where Noval works, are offering paid clinical placements to internationally-educated nurses that could give hundreds of foreign-trained nurses the credentials they need to get licensed. The placements are part of the Supervised Practice Experience Partnership, a partnership between the College of Nurses of Ontario, Public Health and approved partners like Sunnybrook."
"Often, internationally-educated nurses, or IENs, come to Canada expecting to work in their profession but fall short of getting licensed because they don’t meet all of the standards set by the college, which was created to ensure safe nursing practice in Ontario...In 2020, more than 14,600 IENs were pursuing licensing by the college, according to a report by the Office of the Fairness Commissioner. There were another 5,000 or so IENs who were inactive and hadn’t been in touch with the college for the past year. The college says that in the last five years the number of internationally educated nurses that it has registered, or licensed, has increased, from 1,456 in 2017 to 3,235 last year...The hospital wants to identify IENs who are employees, as well as hire more, in unregulated roles such as observers or patient support providers and help put them on a career path. For IENs who are already enrolled in an academic program or working on their language proficiency, the hospital is using funding from the Ministry of Health to offer paid placements in clinical teams in unregulated roles."

Health Workers and the Gender Gap
“Women comprise roughly 70% of the global health workforce, and perform the majority of the sector’s most challenging, dangerous, and labor-intensive jobs. Yet they hold only 25% of the health sector’s senior roles, and are rarely represented adequately in policymaking. Instead, they are often expected to remain passive actors, quietly finding ways to do their jobs in difficult – even impossible – circumstances.”
“Such conditions make essential health-sector jobs unattractive, contributing to severe labor shortages worldwide. Closing this gap is a matter of life and death. For example, midwives are often the difference between safe childbirth and newborn or maternal mortality. ... There is a clear racial dimension to this disturbing trend. ... Black women often report feeling that they are not taken seriously by medical professionals.”
“The health consequences of not listening to women extend further. Children born to healthy mothers are more likely to remain healthier throughout their lives. In West Africa, where I work, qualified personnel are not available at the community level. And I would say there is a disconnect, because at the national level, they know that qualified personnel are not available, but still that is what is in their policies. They require that a qualified nurse or midwife or medical doctor provide family planning services. But they’re not there. I’ve been in villages where there’s not even a nurse or a medical doctor, or a midwife, but a community member who is called “Doctor,” because he’s the only one available. And he’s doing everything—surgery, deliveries, you name it. They’re there, so what are we doing about that? And then you have differences in service delivery between rural and urban settings. All qualified personnel are concentrated in urban areas, and then the most marginalized have no health facility, no qualified health professionals. And we have all those approaches—outreaches, in-reaches—and it has been years, and it’s like we’re starting over and over again. We know where the problems are. But the decision makers—it takes some time for them to reflect on that and make the right decisions.

The women now come looking for me on hearing that I’m around. They want to report their excitement upon taking up a method. I tell them to go to the health center and get counseled, because I’m not the best person to counsel them—so they come back to report their achievement, and they have come to appreciate me. They say, “Thank you. I stopped fearing using a method.” In our questionnaire, there is a long list of methods. You mention all of them to them, and they might say, “Oh, this one I’ve never heard of.” So while being interviewed, they’re also getting to know more about the different methods. So when they go to the health center, they’re able to even mention that “I heard that there is this or that. Can I get to hear more about it?” And that inspires the health provider to speak more about it, and then they take up, they choose. They now have the power to choose.
When people come to me with questions about family planning, I feel so powerful. I feel so inspired. I see where I’m going. I also see where family planning is going, because people have stopped fearing. They’ve broken the stigma, the silence. They’re coming up to me more to report, to give me feedback, even to ask more. So when they’re asking about an area that is not mine, I tell them, “Go to the health facility. We are collecting this information at different levels. I’m collecting at this level, and the health providers are there to listen to you, and also to help you. Go and you’ll be helped.”

First and foremost, I’m a medical doctor. Since my childhood, I’d dreamed of becoming a doctor. Why? Because I wanted to help people, and also because my mother was a nurse, before she retired. I would see that things were not working in my home country. I’m based in the United States, but I’m originally from the Democratic Republic of the Congo. Working in the health sector made me feel good about myself. It was like I was contributing somehow to the development of the country. I started as a clinician on one-one-one consultations, seeing one patient at a time. By then, I realized that it does help people who I would see, but not like a city or a village. So I turned to public health, and then to global health. And my interest in family planning—I would say that started when I was still in medical school.
