How long anticoagulants need to be withheld before surgery :

Dabigatran = 1-2 days

Rivaroxaban = 1 day

Apixaban = 1-2 days

Fondaparinux = 36-48 hrs

LMWH = 12 hrs for prophylactic dose, 24 hours for therapeutic dose

Warfarin= 1-8 days check INR

UFH IV= 4-6 hrs, SC 12-24 hrs

Always check the creatinine clearance:

Drug Duration to withhold before procedure

Dabigatran= 1-2 days with creatinine clearance >50ml/min

Rivaroxaban= 1 day if creatinine clearance >90 ml/min

Apixaban= 1-2 days if creatinine clearance >60
