Columella and Nasal Tip Reconstruction This teenaged boy was playing football and ran full throttle into the goalpost, which resulted in the loss of his nasal tip along with a part of his columella. This had resulted in a disfiguring scar formation with a flattened tip of the nose. He and his parents were subsequently referred to our hospital for cosmetic surgery. Treatment planning was explained to him and his parents and they consented to surgery. The surgery was performed in two steps. The first step involved excising the old scar from the nasal tip region followed by raising a forehead temporal flap, which was then rotated, brought down and sutured to the region of the lost nasal tip and columella. Care was taken to maintain vascular patency in the flap throughout the procedure. Once collateral blood supply had been established to the flap from the nasal tip and columella region, the flap pedicle was excised and the forehead region was sutured. The form of the ala was then refined by slightly debulking it to establish symmetry of the nose.
