when one has alterpathic rot (in a rainworld way). this identification may be because of delusions, alternate beliefs, experiencing matching symptoms in your headspace, etc.
the official wiki says this:
The Rot or Corruption is a type of infection that results in cyst-like outgrowths, appearing in gameplay as a type of environmental hazard and creatures. The Rot is primarily present in Five Pebbles, The Exterior, and Garbage Wastes.
