They are two sides of the same coin, and their conflict suits the struggle Kitana must go through as a character. As someone who had been evil and used by her step-father, Kitana is striving to turn against him and redeem herself. Mileena—created for loyalty and viciousness—is, for all intents and purposes, the evil in Kitana given physical form.
If you want to indulge symbolism, you could say Mileena—with her deformed face—is the ugliness in Kitana.
