Do your loved ones a favour and have this conversation with them. A real, specific, nitty-gritty conversation.
My Mom was quadriplegic and suffered from a serious and deteriorating medical condition. She knew the day would come, and so did I, and we knew in all likelihood it'd be me seeing her through it. So we talked about the days leading up to that day, in detail.
When the days came, I never once wavered or questioned what I understood to be my mother's wishes about every practical thing, because she and I had talked about it. I may have struggled heavily with other things, with other people, but I was *crystal* clear on what she'd say, what she'd think, what she'd want.
Don't leave your people wondering what your interpretation of 'heroic measures' means. Read the document, talk about what life support means, talk about fluids and nutrition and oxygen supports, what they mean, and what happens if they are withheld. Let them know, so when the time comes for them to be making decisions for you, they know.
