I honestly don't think the mission featured in the Secret Level is the same one Titus was sent on in the SM2 ending. It would be unreasonable for Calgar to approve a mission of “absolute” mortality for Titus immediately after welcoming him back.
Or at the very least, he would have communicated the dangers of the mission and allowed Titus to bid farewell to the 2nd Company, rather than making it seem like he was merely borrowing Titus temporarily.
However, it would be interesting if Leandros were the one to recommend Titus for the suicide mission. After all, Titus would once again be clearing doubts about himself—this time from Metaurus. In other words, it's another Titus redemption arc driven by Leandros.

When Titus had just completed the Rubicon surgery, he was planning to return to the Deathwatch. It was the “Brother Chaplain” who stopped him… If “Leandros” had been the one to appear instead, Titus probably wouldn't have stayed even for a moment.
The backstory from White Dwarf #498 mentions that while Titus was incarcerated by the Inquisitor, he received no word from the Ultramarines, leading him to believe he had brought shame upon the Chapter. This ultimately prompted his decision to join the Deathwatch as a Black Shield. (In reality, Calgar had been sending petitions to the representatives of the Holy Orders of the Emperor’s Inquisition to bring Titus back, but all of them were ignored.)
As a Chaplain, Leandros urged Titus to stay, claiming it was so he could prove himself to his brothers. However, in truth, it was also about giving Titus the opportunity to complete his own redemption—to restore his faith in brotherhood.
