1. Dentures, hearing aids, medicine: These are reaaaalllly expensive. Make sure you return ALL of the patient’s belongings. Triple check. Get an extra pair of eyes to look. Have the transporter pick-up people also look. Like seriously, I can’t tell you how much grief lost property can cause on errbody.
2. Advocate for your patient. If, for whatever reason, you/the family/the patient doesn’t feel comfortable getting discharged, discuss with the MD!
3. If your patient is comfort care (has been on a morphine drip) and is gonna go home with hospice or something, make sure you talk with the family and MD about potentially getting the patient off the drip and converting to PO morphine/roxanol/whatevs BEFORE they leave. Ykno, better transition.
