[also hey TW for medical abuse and a kinda graphic description. because like. we can be horny about it all we want but realistically this was a form of medical abuse/neglect IMO and i’ve considered suing over it]

anyway. my first birth was an emergency forceps assisted delivery in the OR because of the size of the damn kid (lol) and a precipitous labor, so that was pretty rough on its own. my contractions were brutal the whole time and he ended up twisted weird so his collarbone was effectively stuck in my hip and it made it feel like my hips were dislocating when i pushed

HOWEVER the real problem was the anesthetic. i am extremely sensitive to most lidocaines and local anesthetic derivatives, and i have a contraindicated reaction to a lot of them (meaning that instead of anesthetizing/numbing, they cause intense burning and nerve pain that doesn’t go away). it was noted in my chart because i had been to this hospital before for all of my prenatal care appointments, so i *assumed* that they would have read the damn patient info and used an Ester (like tetracaine) if i needed local anesthetic for any reason.

since i wasn’t able to receive an epidural, they very unfortunately injected my perineum with LIDOCAINE (ya know, the thing i am very sensitive to) in multiple spots. instead of numbing it caused a horrific burning sensation *in addition* to the delivery pain, and I literally felt the scissors slide in and cut me. 0/10 would not recommend I still remember the sensation vividly

Why did you get an episiotomy? Did the head not fit? Were the shoulders stuck?

During my first delivery, I had an episiotomy because I ended up needing forceps assistance but I also tore pretty badly (despite the episiotomy).

My most recent delivery, I elected to have an episiotomy performed if needed because scar tissue is not very stretchy, and I had a pretty substantial buildup of scar tissue through my perineum from my first birth. It was more likely that I would tear badly again the second time if I wasn’t given a little help, because the scar tissue would have just torn in the same spot instead of stretching like it should.

tldr: tore really badly the first time, and chose to have an episiotomy the second time so that there was less of a chance of tearing again…because unfortunately my partner’s genetics make big babies it seems

sooooo my most recent delivery my episiotomy was performed via pressure (no local anesthetic) because i have adverse reactions to a lot of anesthetics/lidocaine relatives.

basically how it works is they had me push for as long as I could and get her head to start crowning, so then the doctor could use his fingers to pull back on my perineum and cut quickly while i was pushing.

honestly? it was rough but not as awful as i had expected given the last time. when he put his fingers in there was so much general pressure/burning from the head and the extra stretch that it was hard to distinguish episiotomy from ring of fire while i was pushing.

i did still scream tho >~<“

oh yeah, there is a lot of grunting/moaning and a few really good screams in the video. I was pushing as hard as i possibly could basically the entire time (with breaks ofc) and it took about and hour and 10 minutes to push her out after my water broke.

i was definitely exhausted by the end, it took me almost 30 seconds of bearing down hard to push her head past a full crown >~<
