Sam is in disbelief by the thought that Cas was dead, and has to ask him to confirm that it had happened. This isn’t the last time that Cas will choose to die for the Winchesters, but it’s the first time we see Sam’s reaction to it. He is disturbed when Cas confirms it, seeing him as not just an acquaintance, but a friend who put his life on the line for him and his brother.
