This was one of the ideas behind the project: using smart devices with touchskärm to monitor on a constant basis chronic disease patients.

These will improve their quality of life and diminish the necessity for hospitalizations.  Besides, this will allow doctors to continuously monitor patients with the use of such touchskärm devices. To achieve above our project has to integrate different methods and hardware infrastructure to facilitate interaction among patients and doctors.

During the day there are several steps with different time intervals and frequencies for data transmission.  It is transferred only if medical condition of a patient is within tolerable health parameters.

Once the data is collected in such way, our analysts start to analyze and input information into the database. Since analyses are done for three different diseases, three sets of different algorithms are used. The fundamental data needed for heart conditions` treatment is the blood pressure and a heart rate data.  In case of such patients we have processed all the data into special templates.  

After the collected data have been preprocessed for patients and all required input have been saved, a set of machine learning algorithms are used to decide if a potentially risky situation is present. The aim of these tools is to alert a doctor via sms or automated skype call the system has detected worsening of the patients’ conditions.

In case the sms contains a warning message regarding life threatening conditions, the central server should alert the emergency staff or to suggest a doctor to modify the therapy approach. Most of these tools need a preprocessing phase for identifying the correct parameters that need to be validated by the doctors. This means that in the first phaze of our project a large amount of efforts has been dedicated to gather feedback from the doctors about the correctness of the rules / parameters that have been calculated by the algorithms. In this phase another important step has been dedicated by IT engineers to evaluate some probability index for unclear data measures.
