Someone pointed out that book!Grace is only contemplating letting Rocky do major surgery on him to illustrate how much he trusts in Rocky's skill and precision with his craft and while that is correct, especially from a doylist perspective, we are in fact reading Grace's inner monologue; he isn't vlogging in the book. So the heart surgery musings are purely for his own benefit from the watsonian perspective.

And additionally: there is no good expository reason for him to think about Rocky's hands later on, when he's on his way to get him. That's just pure yearning.
