So according to the strategy guide's glossary, Ren had two sisters - one died during the planning stages of Glass Cage, and another one mysteriously disappeared. What if one of the scientists escaped with her because they felt guilty about creating a human who was solely to be used as a tool in an experiment? I wonder what their life would've been like, leading up to the Glass Cage's activation...

but anyways back to your thoughts, if i remember correctly from the game Shin ended up feeling pity (for a lack of a better word) for sai, he didnt like what they had to do to her and in the end actually showing compassion and caring for her (like when he snuck her candy) so it is very likely that another scientist felt pity for the other girls (like shin with sai),for what they were doing and they very well could have kidnapped the one sister, 

as for their life... well they were test subjects, humans made for one purpose and that is to be experiments.  they probably wouldn't have had very good lives, sai even said her life was shit and she wasn't born with the intentions the others were.

i kind of imagine it like growing up in a hospital your entire life because your sick. people acting like everything is ok and pretending to care but really they dont,..im not gonna get into that thought lol

In the end of the game Seto says that he's all alone again. What do you think that to him and Ren?

personally i believe that it means that ren died, in the fragile dreams comic if I remember correctly, ren runs out of medicine, and they have to go back to the tower to get more, i can’t remember if they made it back or not but if they did there is a huge possibility that there was no more medicine, with shin’s AI gone there really isn’t anyone there to mix and make the medicine

the medicine is important because of how she was involved in the glass cage project, if you remember in the game when you go and see sai’s body in her hotel room there are pill bottles, most likely her pills ran out as well and that is how she passed away. or without the pills they (both ren and sai) became affected by the glass cage project the way the rest of the world did and they fell asleep to never wake up.

Between the information in the Fragile Dreams manga and strategy guide, they explained that the process of genetically engineering Ren (and her two sisters)[1] resulted in long-term health problems, and the medication was used to prevent their conditions from deteriorating [2] [3].

They *do* find more medicine for Ren thanks to Shin’s ghost (which lives on in Shiro the cat) [4], but based on the phrasing of the timeline’s events I wouldn’t be surprised if Ren had passed away during their travels, and that by the point in Seto’s life that he’s narrating the story of the game, she has already died [5].

As far as Sai’s need for medicine, it’s not really ever explained beyond several mentions of an endless series of painful tests performed by the researchers. The strategy guide indicates Sai passed away after the Glass Cage activation just like everyone else (well, everyone except those with a severely limited sympathy ability), but the world knew sleeping would bring death so perhaps people at the time took stimulants to stay awake for as long as possible?

The reason Ren wasn’t affected was because she was in cryogenic stasis during the execution of the project [6].

you asked for fragile headcanons so here, consider this; autistic crow and nonbinary seto

ive heard of that and yes i do like that, nothing really says seto had to be a boy, or a girl, plus this is a post-apocalyptic timeline and why would gender even matter. so yeah im diggin. 

and with crow being autistic, with how he acts when you meet him in the park  i cant totally see there being a communication boundary considering he (crow) is a robot and how he perceives humans/emotions/relationships and stuff could be seen as an autitic personality, (idk if personality is the right word to use, i dont fully understand autism mysef but from what ive been told.. ye...)

but yeah anyways, i am digging this headcanon
