After Sal was forced to operate on a criminal with Dante under gunpoint they thought it was over. They were both fresh out of med school and this had messed them up enough, and it kept happening until Dante snapped and shot a man who was holding the gun to his head. With the police and allies of the shot man after them they had to run somewhere. After the incident Dante sat on the staircase of their apartment complex with his head in Sals lap, begging him to go to America.
