When Homelander first debuted as a hero and started doing mini solo missions, he was sent to a terrorist attack site.

Unfortunately a majority of the survivors were killed by said terrorists. Only one person was still left alive, albeit forcefully knocked unconscious.

John rescues the man out of the explosion.
