Quick question cardiophile friends!

Do you prefer strictly a roleplay exam when there are no real issues expected? Or do you like when someone is experiencing real problems, like palpitations, shortness of breath, etc, to ask you to examine them?

For those of you who like to rp doctor exams: do you have a preference for how your patient is initially dressed? Regular clothes? Exam gown? How do you manipulate the patients clothing during your exam?
