In general i just do find stories where homophobia exists more intetesting, but if you are going for a world where it doesn't, you should do some cool fun worldbuilding that fits with that! so this society isn't homophobic, but does it have any particular ideas about human sexuality or sexual orientation? does it view gay and straight relationships in exactly the same way and if so, is that connected to particular family or kinship structures? To ideas about childrearing or inheritance? Does the society have any sexual hang ups unrelated to the genders involved?

Idk i think it's boring and almost kind of cowardly to imagine a typical medieval fantasy world with monarchies and monogamous marriages and primogeniture with the only difference being an inexplicable total acceptance of gay people
