He’s actually never explicitly murdered a child on screen or in the novels, but it is kinda heavily implied due to Akutagawa in general being the most violent mafioso and canonically will kill anyone just to prove himself.
It’s also implied that he killed other children in the slums when he was younger.
TLDR: It's more implied than an actually stated fact.
