In this story both love each other but they keep their affection to himself because one thinks that the feeling will never be returned (izuku) and the other refuses to accept his own feelings, less telling them (katzuki).
In my interpretation, Izuku gave up in his love and took the surgery to stop the pain and do not feel love ever again, on the other side Kacchan keep suffering until the end
