Bilateral Sagittal Split Osteotomy for Long Lower Jaw (Mandibular Prognathism) This teenager complained of having a large lower jaw. She said that this was causing speech and eating difficulties. There was an overjet of 10 mm. It was decided to surgically set back her mandible after decompensation of her arch with fixed orthodontic treatment. Bilateral bone cuts were made in the mandible. The nerve was mobilized with the distal segment thus allowing for free manipulation of the proximal segment. Extreme care was taken to preserve the inferior alveolar nerve throughout the surgery. A slice of bone 12 m in thickness was removed from the region of the bone cuts. Her occlusion was checked and mandible was fixed with plates and screws. She expressed her happiness at being able to eat and speak normally following the surgery.
