Time to reach out to the HR team or the company hiring manager and upskill on your disease knowledge. https://lnkd.in/e6dtadE “The US Food and Drug Administration (FDA) and European Medicines Agency (EMA) have both respectively accepted Novartis’ Kymriah for review as a treatment for adult patients with relapsed or refractory (r/r) follicular lymphoma (FL) after two lines of prior treatment. The FDA also granted the supplemental biologics license application (sBLA) for the CAR-T cell therapy a priority review in this indication. Previously, Kymriah was granted orphan medicinal product designation by the European Commission for FL. The regulatory submission for Novartis’ med are based on data from the Phase II ELARA trial, investigating the efficacy and safety of Kymriah in adults patients with r/r FL. Adoptive immunotherapy has emerged as a promising cancer treatment, with the body’s immune cells engineered and cultured ex vivo to generate a ‘living drug’, given to patients to specifically target and kill malignant cells. 

Compared with radiotherapy and chemotherapy, the advantages of immunotherapy are that engineered T cells provide a potent immune response to eliminate the tumour, the cells continue to survive in the body, and contain immunological memory to react to tumour relapse. 

Clinical trials using engineered chimeric antigen receptor (CAR) T cells against blood cancers have been remarkably successful, showing response rates greater than 90% for acute lymphoblastic leukaemia. The long-term benefit is seen in patients such as Emily Whitehead, who was treated at Children’s Hospital of Philadelphia and has since been disease free. 

However, not all patients respond in the same way, and one of the challenges when making comparisons lie in the diversity of T cell populations reinfused to patients. A study at The Fred Hutchinson Cancer Research Center attempted to address this issue by manufacturing a defined ratio of CD4:CD8 T cells. The scientists were able to show correlation between cell dose and CAR T cell expansion, and a similar high remission rate to other studies. 

Another manufacturing strategy has been to preserve a less differentiated phenotype during T cell expansion, as these central memory or naïve T cell subsets have shown a higher potential for engraftment and a greater anti-tumour capacity.
