I think in this world centaurs have a uterus in the top and horse half. A pregnancy starts in the top, human half before moving down into the horse half (with all the pain of a normal birth). Once that happens a centaur can get pregnant again with the top uterus free.

K up there is wearing a a sort of binder because he's embarrassed about the post-partum belly. He's also not happy about not being able to fit through doors anymore.
