it should go without saying, but sai was raised with the idea that he is a weapon of war, emotionless and apathetic. he had no concept of his worth or any sort of ego. regarding its functionality, he is satisfied with his body’s performance and maintains it with a specific training plan and diet. his goal is to maintain muscle, burn fat and replenish his chakra between missions. regarding its practical use, he feels that his body serves a purpose, even the imposed idea of handsomeness which is often lauded unto him.
finally, for the longest time, sai cared little for being attractive. still, he kept his hair cropped neatly, wore fashionable clothing, applied makeup when necessary. for anbu, who primarily engage in stealthy missions where direct planting is necessary, beauty is yet another tool to be utilized. hence, why he’s trained to smile no matter the adversity. in short, attractiveness was yet another measurable trait to be utilized towards his skillset.
later in life, sai undoubtedly goes through a period of disassociation with himself, his actions and his physical body. however, this runs concurrent with the rediscovery of emotions, memories and other human traits which were sealed away by training and trauma. however, he overcomes this and alongside the development of his own self - worth, a confidence also blooms in his abilities, his body and his looks. still, if he were to ever have a significant other, for example, he’d keep himself well - groomed for them, not for himself. that self - sacrificing nature never really leaves him.
