What is "Case Management"?

The person who helps the individual and/or his/her family to develop a client-specific plan of care is called a case manager. He or she also works closely with the individual and his/her team to ensure satisfaction of services, monitor health, safety, and welfare, and support progress toward identified goals and outcomes.

Our agency is currently accepting individual referrals for conflict-free case management through the Supports for Community Living and Michelle P Medicaid waivers, as well as private-pay services.

Our agency is happy to help ensure this for the individuals we serve by providing conflict-free case management. Essentially, we work together with our individuals and their teams in a non-biased manner, putting people first and focusing on goal achievement.
