FDA Indications/Dosages:

For the relief of the symptoms of bladder instability associated with voiding in patients with uninhibited neurogenic or reflex neurogenic bladder: One 5 mg immediate release tablet given 2-3 times a day. Maximum of 20 mg per day.

For the treatment of overactive bladder with symptoms of urge incontinence, urgency, and frequency: Start with one 5 mg extended-release tablet daily. May increase at weekly intervals to a maximum of 30 mg per day.

Not recommended for children under the age of 5 years.

Pharmacology/Pharmacokinetics: Oxybutynin inhibits the muscarinic action of acetylcholine on smooth muscle. Blockage dies not occur at skeletal neuromuscular junctions or autonomic ganglia. Oxybutynin chloride relaxes bladder smooth muscle which decreases urgency, frequency, urinary leakage, and dysuria. Extended-release tablets contain oxybutynin in a tablet comprising an osmotically active bilayer (drug layer and push layer) core surrounded by a semi-permeable membrane. Water from the GI tract enters a laser-drilled orifice in the semi-permeable membrane causing the drug to go into suspension and push layer to expand. Drug is pushed out the orifice at the constant rate over 24 hours. Peak plasma levels are reached in 4-6 hours after an oral dose and remain steady for 24 hours. Mean terminal half-life of oxybutynin is 13 hours. Steady state levels are reached by day 3 or oral dosing. Metabolism occurs via CYP3A4 in the liver and gut wall.

Drug Interactions: May decrease the effects of phenothiazines and haloperidol. May cause tardive dyskinesia when used with haloperidol. Additive effects may be seen with other anticholinergic agents.

Contraindications/Precautions: Contraindicated in uncontrolled narrow-angle glaucoma, urinary retention, and gastric retention. Use with caution in patients with renal or liver impairment, gastrointestinal obstruction, ulcerative colitis, intestinal atony, and myasthenia gravis. Use with caution in patients with gastroesophageal reflux disease and in those taking drugs that can cause or exacerbate esophagitis (such as bisphosphonates). Heat prostration can occur in the presence of high environmental temperature. Pregnancy Category B.

Adverse Effects: Dry mouth, drowsiness, confusion, agitation, blurred vision, diarrhea, and nausea. Side effects may decrease with prolonged use.

Patient Consultation:

Avoid prolonged exposure to hot weather.

May cause drowsiness. Use caution while operating machinery or when mental alertness is required.

Store in a cool, dry place away from sunlight and children.

If a dose is missed, take it as soon as possible. If it is closer to the time of your next dose than the dose you missed, skip the missed dose and return to your dosing schedule. Do not double doses.

Contact a physician if the above side effects are severe or persistent.

Swallow extended-release tablets whole. Do not crush, split, or chew.

The semipermeable membrane tablet shell of the extended-release tablet passes through the GI tract intact and may be found in the stool.

It's been a while since I posted but I thought I'd share some experience with some medications I've tried with dealing with interstitial cystitis. After my cystoscopy they gave me vesicare which has been beautiful. I'm not saying it's. Magic pill but after 2 months I'm very grateful. UNFORTUNATELY it's not covered by most if not all insurances because it's fairly new. I was recently hospitalized and they gave me ditropan and it was the WORST. dry mouth to the max. I was in taking so much water but still no water wasn't enough. Anyone else have that experience with that medication? anyways, I feel as though my IC has been manageable. Hasn't been cured or gone but I've been reducing my intake of caffeine and acidic foods. That's all we really can do right? Hope the world is doing well ❤️
