The Medicare CDM Scheme (formerly Enhanced Primary Care) provides rebates for physiotherapy treatment for patients who are referred by their GP under a GP Management Plan or Team Care Arrangement. Eligibility for this service is based on your GP’s clinical judgement, taking into account your medical condition and your complex health care needs.
The program allows for up to 5 rebated consults with allied health professionals, including physiotherapists, podiatrists and dietitians, in a calendar year. Physiotherapy sessions are only one of the allied health professional services offered under this scheme, and you will need to be referred by your GP for all other allied health consultations.
Qualitative data indicated that the ability to engage in chronic disease self-management was influenced by numerous personal, environmental and system factors. These factors were not reflected in existing CDMP programs and may explain why they do not effectively meet the needs of socially complex participants. This suggests that a community-based approach to CDMP may be more effective than individualistic approaches to implementing a CDMP for socially complex populations.

The CDM service (previously GP management plans and Enhanced Primary Care or EPC items) provides Medicare-subsidised appointments with allied health providers such as physiotherapy, osteopathy and podiatry to patients with complex chronic conditions. This is done through a referral from your GP.
Patients with a chronic health condition who meet the criteria for GP Management Plans (GPMP) and Team Care Arrangements (TCA) can receive Medicare rebates for up to five private allied health services per year. GPMPs and TCAs are designed to help improve outcomes and reduce the need for emergency and potentially preventable hospitalisations.
GPMPs and TCAs help to coordinate care between a patient’s GP, specialist and allied health providers. GPs prepare the plan and write referrals to a number of allied health professionals including physiotherapists, speech pathologists, psychologists, dieticians and podiatrists. The GP also reviews the progress of the care and reports back to the patient.
The Medicare physiotherapy rebate covers up to five TCA sessions each calendar year and resets on the 1st January. Getting Medicare rebates for physiotherapy can be a bit tricky. There are certain criteria that you need to meet in order to qualify for Medicare physiotherapy.
To be eligible for a Medicare Care Plan your GP needs to refer you to an Allied Health professional such as a Physiotherapist, Podiatrist or Chiropractor. Typically, your GP will refer you for up to 5 individual Medicare-subsidised sessions with your chosen Allied Health provider per calendar year. Depending on your particular condition, you may be referred to multiple Allied Health professionals. For example, if your condition is chronic you may be referred to a Physiotherapist and also a Podiatrist or Chiropractor. This will allow you to get a complete team approach to managing your condition and improving your quality of life. The Allied Health providers will work closely with your GP to manage your conditions.
