I'm scared of having DID. I'm pretty sure I do and I am scared of how it will affect my life, having to tell people, I am just very scared. :(
So okay, it affects you. Don't worry too much about how it will affect you - because if you have it, it affects you now, and the way it does isn't likely going to change all that much, y'know? And I'm sure while there's definitely negatives - you wouldn't be thinking it's DID if there weren't - it's not all bad, and it's no guarantee that you all can't still have a good life. There's a ton of people out there with it who are enjoying life, including all the normal things like jobs and family and friends and hobbies, despite having DID.
And second, you never have to tell anyone. Ever. I mean with a couple obvious exceptions, like it might be a good idea to tell someone if you're planning on marrying them and living with them for the rest of your life, and if you're seeing a therapist for help with it, it's probably best to tell them too... But other than that, in general? No one ever has to know. You're not obligated to tell anyone.
It's also okay to feel scared. There's a lot of uncertainty in your future, and feeling scared about that is normal and natural. It means you don't feel ready and capable. And that's okay. You don't have to be ready and capable right now. You can become so a little bit at a time. And honestly, like most people, you're likely not going to be truly ready and capable until after you've challenged things a time or two first. There's things you have to grow confidant and comfortable with doing, by first doing them when you're not yet comfortable, you know?
And that's okay. It's a learning process for all of us. You all'll get there, you'll figure out what you want from life and how you all will get it, together, in time.
Have faith in yourselves. It won't be easy, but usually nothing worth having is easy. And you're stronger, tougher, smarter, more capable than you think.
It's also okay to ask for help from social services. There's no shame in needing help. I'm not really sure how to help with getting that, except to recommend you search "social services [location/area]" and go from there.

You're gonna be just fine. Whatever challenges life has to throw your way, you'll get through them together.
