This is a very aggressive injury. Therefore, we should NOT do the incisional biopsy because it will increase the chance of metastasis, besides promoting an inflammatory process, which will lead to increased safety margin. Melanoma is an exception (between pigmented lesions) to which you should AVOID incisional biopsy. The treatment is surgical, with margin of safety and removal of the regional lymph nodes. Embora, a maioria ocorra na pele, essas lesões podem se desenvolver em qualquer sítio em que os MELANÓCITOS estejam presentes. Esta é uma lesão muito agressiva. Por isso, NÃO devemos fazer a biópsia incisional, pois ela estará aumentando a chance de metástase. Sem contar que isso vai promover um processo inflamatório, o que levará ao aumento da margem de segurança. O melanoma é uma exceção entre as lesões pigmentadas ao qual você deve EVITAR a biópsia incisional. O tratamento é cirúrgico, com margem de segurança e remoção dos nódulos linfáticos regionais.
