Here's what I think: the show doesn't need a female doctor. It needs a female writer. Under Moffat especially, a female doctor would be nothing but a bunch of jokes about boobs and periods, and it wouldn't do ANYTHING for the show, for women, or the character. The majority of fans would probably be pissed off, and the Doctor himself (see, male pronouns) has identified as male for the whole show - when DT regenerated into Matt Smith, he was shocked at the long hair and seemed concerned about being a woman. (I don't have a link but you can find it on youtube). So yeah, possible for Time Lords to switch gender, but unlikely and probably uncomfortable for the character, not to mention the blatant sexism that would occur if Moffat was to write her.
