Belly flopping onto the beach at near terminal velocity because you confused the air break on the parachute with the buttons for quickly looking behind you. Thus failing the heist at the very end.

Flying into a missile that is homing onto the jet attacking you, thus causing the current heist to fail.
GTA FAILURE #2
Getting the mission required vehicle stuck in such a way that it promptly explodes, failing the mission just before completion.

Walking into the tail rotor of the getaway helicopter just before completing a heist.
