After escaping prison, Dream can't seem to stay warm. He is freezing all the time and cold to the touch no matter what climate he is in. Cold is stuck in the depth of his very bones, his blood, like he were dead.
It doesn't change, although, to be fair, he has not been doing anything about it, assuming it would be pointless anyway.
Even months after the prison, the cold remains within him, ever present, assumed permanent. It makes him less precise, easier to evade and attack, movements stiff and painful. When he encounters Tommy after months of inactivity on his part, Dream is beaten easily. But Tommy does not kill him after that.
No, in fact, Tommy does quite the opposite. He takes Dream home with him.
He makes comments on how cold Dream iss, like Dream isn't perfectly aware of that fact, and he makes Dream sit in front of the large fire in the fireplace, wrapped in blankets, and feeds him hot soup.
It doesn't seem to work to get the cold to leave him, but Tommy is not giving up.
He drags Dream into the bathroom and draws a bath for him. The tub fills quickly with hot water that Tommy makes Dream get into, before following him to Dream's surprise and confused mortification.
The bath makes him feel warm for the first time in forever, but that's woefully shortlived. He notices quickly that the warmth is only on the outside and refuses to seep into his core, refuses to chase away the freezing cold within his bones and blood and organs.
Tommy notices Dream's frustration, the tears in his naked eyes, his mask long since abandoned somewhere. For some reason that Dream cannot fathom, Tommy wants to help him. It makes no sense to him, not after everything, but he's too tired and frustrated to really think about it.
Tommy asks him if he is still cold, to which Dream replies affirmative, tells Tommy that he is still cold within. Tommy looks down towards Dream's stomach, who shakes his head at how literal Tommy is taking his words, but he can barely do that, before startling back, because Tommy is quickly coming towards him, into his space, until they're mere inches apart.
He asks Tommy what he's doing, and Tommy tells him he thinks he knows a way to make him feel warm inside. There is a look in his eyes that makes Dream's cheeks grow hot-- earnest and determined, solely focused on Dream.
Dream can barely nod before Tommy's lips are on his, the boy's hands roaming over Dream's body, somehow hotter than the water around them. His mind is melting with the heat, growing hazy and sluggish and pliant in Tommy's warm hands that are pulling him closer, exploring his body, always exactly where Dream wants him.
It's dizzying, all that warmth, almost too much, but it does not yet reach into him. That's when Tommy spreads Dream's legs. He is not met with resistance, neither when spreading his legs, nor when he spreads Dream elsewhere, drawing slurred moans out of Dream.
The hot water makes it a bit strange, not quite comfortable but not unpleasant when Tommy finally enters Dream.

And something seems to snap and crack and spill forth within Dream, like a chain reaction and Tommy is the catalyst. There is warmth, sudden and blessed, bursting to life within him, a fire that starts in his stomach and spreads throughout his entire being until all of him is burning up from the inside out.
He cannot move, cannot even think, that's how good he feels. He is but a melted puddle of his former self, held together only by Tommy's hands and arms, cradled lovingly, which only makes it all worse-- better.
The tears can't be helped. He is finally warm again.
