Just put it in your mail like that, if you live in Japan (the brackets are just the translation, please put in the JP stuff). I think the mail can be sent overseas as well (as once I sent via a friend a mail to Kusanagi Mizuho, which she answered kindly, even though it’s kind of an anon mail) and fan letters are usually sent to Hana to Yume’s editorial department, which is located in the address above. Hoping all goes well and you can mail her a fan letter, anon!
