I think I've figured this out. While Polnareff and Kakyoin were watching the Hermit Purple affected T.V. when it was on, there was a point when it was off because they were carrying it to somewhere more private. We know Kakyoin can see through his stand because in the Strength episode, he used it to search the ship. Before the The Lovers escaped from Joesph's brain, Hierophant Green attached a thread of itself to The Lovers so when it entered the little girl's ear, Hierophant Green restrained it. So I think Kakyoin was seeing what was going on in Joesph's head through his stand as well as what was on the T.V. while Polnareff would watch the T.V. when it was on and Hierophant Green would guide Silver Chariot with a thread wrapped around its wrist or something when it was not.
