I believe most piglins give birth to dozens of babies, but due to their anatomy they can't handle all of them to full term, so they are usually born undercooked.
But worry not: the nether's weather and air condition is the perfect place for premature babies to grow. It's humid and warm, almost resembling a mother's womb. The babies grow just find just wrapped in blankets, held close and fed regularly. They're quite fragile, however.
Mothers usually give birth to around 3-6 piglets, but usually only 1-2 of them survive the first three years. Toddler piglins are gifted a temporary name at age 3. When their immune system is strong enough for them to survive without their mother's anti-biotics (transferred with their milk.)
