Common Dosage Forms:

Tablets, extended-release: 25 mg, 50 mg

FDA Indications/Dosages:

Treatment of overactive bladder (OAB) with symptoms of urge urinary incontinence, urgency, and urinary frequency: The recommended starting dose is 25 mg once daily. After 8 weeks, the dose may be increased to 50 mg once daily based on individual efficacy and tolerability.

Dosing in patients with severe renal impairment (CrCl 15-29 mL/min) or patients with moderate hepatic impairment (Childs-Pugh Class B): The maximum dose in these patient populations is 25 mg once daily.

Pharmacology/Pharmacokinetics: Mirabegron is a beta-3 adrenergic agonist. Stimulation of the beta-3 adrenergic receptors in the bladder relaxes the detrusor smooth muscle during the storage phase of the urinary bladder fill-void cycle. This results in a greater bladder capacity. Mirabegron is well absorbed after oral administration. Administration with a low-fat meal decreased Cmax and AUC up to 75% and 51% respectively. Metabolism occurs via multiple pathways with excretion primarily through the urine.

Drug Interactions: Mirabegron is a moderate inhibitor of CYP2D6. It may increase exposure to drugs metabolized by CYP2D6 (metoprolol, desipramine, thioridazine, flecainide, propafenone). It may also increase digoxin levels. Urinary retention may occur when used with antimuscarinic agents.

Contraindications/Precautions: Not recommended for use in patients with severe uncontrolled hypertension (SBP ≥180 mmHg, DBP ≥110 mmHg), in patients with end-stage renal disease (ClCr <15 mL/min) and in patients with severe hepatic impairment. Use with caution in patients with bladder outlet obstruction, in patients taking antimuscarinic medications for overactive bladder, and in patients with severe renal impairment or moderate hepatic impairment. Use with caution in nursing mothers. Pregnancy Category C.

Adverse Effects: Common adverse effects include hypertension, urinary tract infection, urinary retention, headache, and nasopharyngitis. Atrial fibrillation, angioedema, and prostate cancer are serious but very rare adverse effects (<0.2%).

Patient Consultation:

Can be taken with or without food.

Swallow tablets whole, do not crush, divide, or chew.

Contact a physician if the above side effects are severe or persistent.

Store in a cool, dry place away from sunlight and children.

If a dose is missed, skip it and return to normal dosing schedule.
