Yknow how people tend to play as the opposite gender in video games, and how since in lore its like the year 2138 so the voice changer mods are probably quite advanced.
Wouldn't it be possible for Ainz to have roleplayed as a man in the beginning then it became basically imposible to own up the truth?
Like if heteromorphs were already being targeted maybe she didn't want to add being a woman into the mix? Or maybe she just wanted to have fun? Doesn't really matter why she started, just that the entire guild knew her as "Momonga, the reliable leader of the guild"
Somehow no one figure it out or even cared about it enough to mention it, probably cuz a 'guy' who uses a voice changer and is maybe not do tradicionally masculine wouldn't be a big deal compared to the rest of the group, they were woke or something so it didnt even bother them. The NPCs just had even less information compared to their creators so they were also in the dark about it.
Nyways, I imagine this DOESNT translate into the new world with their medivial society, they don't know Ainz as a regular salary worker, they know Ainz as the King of the Sorcerous Kingdom, so they don't have like positive feelings which allowed the guild and NPCs to ignore Ainz's behaviors that don't align with a cisman.
I'm just imagining someone like Ainzach, which is one of the only new worlders who Ainz has a semi normal relationship with, slowly but surely realizing "Hey... this guy has some interesting behaviors, maybe its because he is an undead?" and he asks Ainz thinking he will get some answer reacting to, yknow, Ainz being a powerful undead or a ruler of a monstrous kingdom and shit.
"Oh, I'm technically a woman but I have a kingdom to rule so being a man is more successful when it comes to negotiating" Ainz simply replied, as if it was common knowledge.
Which... didn't answer the original question, but was useful? Maybe? Like it did explain Ainz's obvious staring of dresses and other traditional feminine stuff, but like does gender make a difference if you're a powerful undead? When the two of the talk, should Ainzach use she/her or he/him or some mysterious third or fourth option?
Anyways, this leads to Ainzach being the second person to know Ainz's original gender, first being Pandoras actor as Ainz probably didn't care about using his voice mod when he was a) organizing the treasury b) the last person on the guild. And so the PA became the first person to know this side of Ainz.
Fast forward to like maybe some months later, Ainzach somehow became a close acquaintance of the Undead ruler of his nation since, for some reason, the undead is asking HIM (out of all the probably better options, he doesn't really how many male friends his king has but theres probably way more appropriate options rather than a guild leader) random questions about how human perceive men (Ainz just trying to compare gender standards in his era vs the new world, to see if she should change her behavior) and Ainzach somewhat becomes Ainz's new person to look at when she's trying to learn new world mannerism and whatever male questions she may have.
