they didn't change anything huge about it other than the visuals and the music (now remixed by danny b, composer for super meat boy, and a dude who was involved in the big cave story remix album from a while back)
a few levels got expanded though (egg corridor's revisit, the labyrinth, the outer wall), and there are a few prinny-related easter eggs since it was published by nippon ichi
really the best thing about the game to someone who loves cave story as much as i do is the redesigned world. it's just so much more atmospheric, and the island feels like a very real place with a mysterious past and living inhabitants more than ever before.
the only huge problem is that all the cutscenes are scripted exactly like they were with the sprites, so the movement of characters during cutscenes is really odd with the otherwise far more realistic presentation
so, i mean, i really loved it, but cave story is my second favorite game of all time so i'm PRETTY BIASED
